Authorization for estimate and dismantling
I authorize Moberly Motors to prepare an estimate of repair costs and if necessary dismantle parts of my vehicle to do so. I understand that this is not an authorization to repair and is only for evaluating damages for myself or the insurance company covering the cost of repair. I understand that the prepared estimate will contain my personal information and approve its contents to be transferred to any persons or businesses needing the information in order to process my claim.
Moberly Motors will not be held responsible for items left in any vehicle either towed or driven in estimating or repairing. Any labor involved in dismantling or estimating must be paid before vehicle is moved from Moberly Motors premises. Vehicles towed or driven in, then deemed a total loss, or moved to another location for any reason by customer or Insurance Company may be subject to admin, lot, debris cleanup charges, storage or estimate fees. Any labor, towing or lift inspection fees must be paid before a vehicle leaves Moberly Motors. Outside Storage $25 - Inside Storage $50 Per Day
I hereby authorize all estimated repair work, including installation of necessary parts, materials and equipment as may be reasonably necessary to alleviate the complaint set forth herein. I hereby agree that I shall pay your customary price for such labor, parts, materials, and equipment. All sums due you shall be paid immediately upon completion of the necessary repairs; any unpaid balance shall bear interest at the rate of 24% per annum computed from the date of such repairs. In the event I fail to pay any such amount due to you I agree that your costs of collection, including court costs and reasonable attorneys' fees shall be recoverable from me.
I hereby agree to pay a 25% restocking fee for any parts returned if I do not have my vehicle repaired.
I hereby agree that you are not responsible for loss or damage to the vehicle or articles left in vehicle in case of fire, theft, or any other cause beyond your control or for any delays caused by unavailability of parts including vehicles left in our care without sufficient antifreeze
I hereby grant you and/or your employee's permission to operate the vehicle herein on streets, highways, or elsewhere for the purpose of testing and / or inspection.
An express mechanics lien is hereby acknowledged on the vehicle to secure the amount of repairs thereto. I hereby waive my right to a written estimate of the extent, nature and cost of the services to be provided.
Power of Attorney:
Know all men by these presents that I (or we), the undersigned, do hereby constitute and appoint Moberly Motor Co. as my (or our) true an unlawful agent and attorney-in-fact to endorse in the name, place and stead of the undersigned the check of draft issued by the insurance company to cover all or part of repairs to my (or our) automobile which are authorized b my (or us) in whatever manner in seeded to negotiate such a check.
Rental car Policy: Moberly Motor Cs. is not responsible for any rental car charges. All rental car charges are the responsibility of the customer and their insurance company
